Appointments can be made by calling our office during normal business hours of by submitting an appointment request here.
We ask that all patients arrive 10-15 minutes before their scheduled appointment time in order to complete any necessary paperwork.
We have a 20-minute grace period for all appointments. If you are late you may be asked to reschedule to a later time or a later date so that the patients who are on time are not inconvenienced.
Appointment times are approximate times only. Because each patient requires individual attention by the doctor and her staff for whatever time period that may be necessary. The doctor, as a specialist, is also confronted with emergency cases during the day which could delay the scheduling times in our schedule. If this happens we always try to contact our patients to come in at a later time or give the option to reschedule at their convenience. Please try to understand if you are asked to wait for the doctor.
Cancellation & No-Show Policy
Failure to give at least 24 hours advance notice to cancel or reschedule your appointment will result in a $25 charge to your account. This broken appointment fee will need to be paid on or before your next office visit.
Co-Pays & Insurance
We accept most local and many national insurance plans.
Co-pays must be paid at the time of service, even with secondary policies. If you do not have your co-pay with you, you may be asked to speak with the Office Manager to arrange payment. New patients must have co-pays on date of service.
We ask that you provide complete and accurate information about your insurance so that we may submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance or other deductible amounts.
It is your responsibility to know what your insurance provides. You insurance may have limits on the doctors you can see or what services you can receive. Please read your policy carefully and know its provisions, as well as its exclusions. The doctor and staff cannot be knowledgeable about the coverage of the hundreds of health care plans presented.
We accept cash, checks, and most major credit cards and debit cards. We DO NOT accept personal checks.
Prescriptions are E-faxed to the pharmacy after your visit and it is your responsibility to pick it up at pharmacy.
Contact your pharmacy to inquire if it is ready as some prescriptions may take 4-5 hours to fill.
If you have a Mail Order prescription program through your insurance, we will mail or fax the prescription upon your request.
If you insist on having a prescription called in and it is not an EMERGENT prescription, we will call it in within 24 hours.
